LPN Care Coordinator M-F 40 hours days Job at Comfort Keepers of St. Cloud, MN
Comfort Keepers is a locally owned and operated licensed home care franchise since 2004. With over 800 Comfort Keepers franchises, we are growing and are ranked in the Top 10 overall. This is a Non-medicare and non-medicaid company. Comfort Keepers is looking for another LPN to join our Brainerd Lake Territory..
This position will work M-F 8am-430pm. No weekends, evenings or holidays.
LPN will conduct weekly Skilled Nurse Visits, Case Management Communication and Documentation, and Fill in with HHA and HMKG shifts as needed to support call offs if schedule allows. The LPN will also spend time in office assisting with new hires, trainings, documentation and supervision.

Requirements
- LPN in the State of Minnesota
- Drivers License and Clean Driving Record
- 12 months experience as a LPN in Home Care or LTC, preferred
- Comfortable learning new software and electronic charting
