Responsible for providing health counseling, first-aid, medical care, information and referral services, both on site and during field visits to the Cornerstone RRR / Homeless Continuum. Provide instruction and direct services in prevention health measures, including immunization and communicable disease investigation and case follow-up. Assist in developing program protocol in securing health and medical services for the homeless, mentally ill to enhance quality of life and increase over-all client satisfaction and self-determination to meet independent living goals.

Supervise day-to-day health care needs of program clients and provide basic medical and emergency first-aid care either on site or in field as applicable. Provide direct supervision of physically ill or endangered clients as applicable.
Coordinate medical appointments with clients, physicians, and treatment personnel as applicable. Responsible for dispensing, follow-up, observing and reporting client’s behavior and/or response to prescribed medication, and documentation of medication services.
Provide medication support groups to assist clients in stabilizing, compliance and/or side effect either on site or in field as needed.
Assist psychiatrist on an as need basis in transcribing orders, dispensing medication and providing appropriate nursing care to program clients. Coordinate with multi-disciplinary treatment team in carrying out nursing functions and education in enhancing quality of life and physical/mental health of clients during daily team dispositions.
Coordinate and gather any and all medical records from previous/outside treatment providers. Ensure consent for treatment forms, schedule physical exams, assure receipt of immunization records (as applicable), maintain updated Medi-Cal documents and current eligibility.
Review, order and stock health education material and keep current literature available for client information and knowledge.
Coordinate transportation services for clients to obtain needed health care services.
Attend relevant community meetings to network and brokerage with other health and service providers in locating health services for client population in meeting their health/medical needs. Secure providers for targeted population, including but not limited to vision, dental purposes and find alternative low cost health prevention service providers.
Provide hygiene and general health care / maintenance instruction to program clients.
Conduct and coordinate staff training pertaining to medical issues and maintenance of a regularly scheduled CPR, Red Cross Aid, and AIDS/HIV training regimen.
Maintain client medical records and files including medication documentation, lab work; and all other necessary health and medical documentation requirement.
Keep up to date, accurate records for HUD, U.S. Department of Housing and Urban Development for annual reporting.
Perform other duties as assigned by Program Manager, and/or Assistant Director.

Qualifications

EDUCATION: Minimum of an Associate of Arts degree.
EXPERIENCE: Minimum of one year experience in a mental health treatment setting preferably with homeless mentally ill adults.
LICENSE OR EQUIVALENT: Licensed in the State of California as a Licensed Psychiatric Technician and/or Licensed Vocational Nurse.
ADDITIONAL REQUIREMENTS:
Knowledge of psychopharmacology, mental health or related programs, treatment theory and techniques for mentally ill and /or dually diagnosed adult population.
Ability to communicate and work with an extremely mentally disturbed and ethnically diverse population; work successfully within a multi-disciplinary treatment team. Ability to perform detailed work with a high degree of accuracy.
Must have or obtain Red Cross First Aid and CPR training. May required obtaining Crisis Prevention Management training.
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Must successfully complete the Center’s HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.
Regular attendance is an essential function of the job.
Must have a current California driver license and be insurable by the Center’s auto insurance carrier.
Interacting with other employees at work is an essential function of the job.
Arriving at work on time and not leaving early is an essential function of the job.
