Position Summary
At Stony Brook Medicine, the T&R Center Licensed Practical Nurses provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.
- Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
- Participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family.
- Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
- Possesses excellent communication skills; is skillful in mentoring and teaching; and participates on committees or projects.
Duties of a T&R Center Licensed Practical Nurse working for the Centralized Throughput Office include the following but are not limited to:
- Plans and manages patient care according to each patient’s needs.
- Interviews patients and records their medical history and physical condition.
- Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
- Provides routine care for patients.
- Observes patients’ health.
- Reviews medical charts.
- Accompanies patients to their rooms.
- Assists physicians and registered nurses.
- Communicates with patients and their families.
- Performs standard diagnostic procedures including EKG, phlebotomy, venipuncture, PFT, and Doppler.
- Prepares specimens for lab courier.
- Administers medications.
- Provides wound care.
- Prepares and administers injections and/or enemas.
- Assists with patient comfort.
- Monitors catheters.
Assist the primary nurse in initiating/providing needed interventions, medications and therapies, physical assessment, admission documentation, and basic care to the admitted patients in the Emergency Department.
Provide comprehensive report and expedite rapid and safe transfer of admitted patients in the Emergency Department to their assigned floor at the direction of the Centralized Throughput Office.
Working Conditions:
Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue as well as other unpleasant elements such as accidents, injuries and illness. May be exposed to and be required to handle varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries. This position is classified by OSHA as Category 1.
Qualifications
Required: Must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State.
Preferred: Acute care*, phlebotomy, EKG, and/or IV experience.
- Acute care is defined as medical services provided for illnesses that are short-term (usually less than 30 days), intense, and have a relatively severe to very severe course, requiring knowledge and expertise in the vigilant monitoring of subtle changes in the patient’s condition, as well as ability to quickly recognize and identify potential problems and conditions resulting from a previous disease.

In accordance with the New York State Department of Health (DOH) regulation that all hospitals and nursing homes “continuously require all personnel to be fully vaccinated against COVID-19,” Candidates who are not already partially vaccinated must obtain the first dose of the vaccine within three (3) calendar days of acceptance of conditional job offer and must obtain any subsequent doses in accordance with the vaccine protocol Candidates who are partially vaccinated, but not yet fully vaccinated, must complete their vaccination series within three (3) calendar days of job offer or in accordance with vaccine manufacture protocol, whichever comes later.
The regulation also includes those who may be affiliated with or interact with employees of a hospital or nursing home. The regulation allows for limited exemptions with reasonable accommodations, consistent with applicable law.

- Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*
- Complete electronic reference check with a minimum of three (3) professional references.
- Successfully complete a 4 panel drug screen*
- Meet Regulatory Requirements for pre employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).
Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
