The primary purpose of your job position is to provide direct and comprehensive nursing care to the patients, supervise daily nursing care and services, and provide leadership to nursing staff. Your duties include but are not limited to assessing patient needs, planning and implementing patient care activities and evaluating patient’s response to treatments. Patient care is delivered in a collaborative manner with the patient, family, and all the members of the health care team. You will also manage activities within the therapeutic milieu and direct the delivery of nursing care by other members of the nursing treatment team. The supervision of staff and care must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Nurse Manager or Executive Director to ensure the highest degree of quality care is maintained at all times.

QUALIFICATIONS

Must be at least 21 years of age.

Successful completion of State-approved training course & education requirements for registered nurses (i.e., diploma, associate’s degree, or BSN in Nursing).

Must have an active, unencumbered Tennessee Registered Nurse license

Must be CPR/BLS certified or able to obtain certification prior to working on the floor.

Ability to maintain cooperative working relationship with patients, patients’ families, and medical staff, as well as facility personnel. Possess leadership qualities.

Excellent communication skills, ability to make independent decisions when circumstances warrant such action.

Advocate of team concept, with willingness to contribute and be supportive.

Able to work 40 hours per week and other duties as needed by the facility.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Complete the initial nursing assessment within 8 hours of admission. Complete an assessment for withdrawal symptoms upon admission within a reasonable amount of time and on an ongoing basis.

Reassess the patients on an ongoing basis, based on patient needs, interventions, and expected outcomes and is documented.

Participate in and receive the nursing report upon report for duty. Report any abnormal changes in patient health promptly. A clear, concise, and accurate report based on the nursing process is reflected in the patient record and change of shift report.

Make daily rounds of the unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report any findings to the Nursing Manager. Notifies attending physician and family when the patient becomes ill or is injured.

Participates in the multi-disciplinary care planning and implementation to meet the physical, psychological, psychosocial, and spiritual needs of the patient and their family.

Initiate the development of the interdisciplinary plan of care and manages nursing actions/interventions to address the patients ongoing and long-term needs based on the nursing assessment, the DSM diagnoses, strength and weaknesses, estimated length of stay, discharge plan, problems to be addressed, and measurable goals with a target date and interventions.

Develop work assignments, and/or assist in completing and performing tasks. Assist in developing methods for coordinating nursing services with other patient services to ensure adherence to prescribed plan of care.

Provide nursing care based on the standards of care, assist the patient to achieve maximum psychological and health capabilities and promote achievement of expected outcomes at discharge.

Evaluate patient’s actual and/or potential response to the disease process, interventions and the outcomes of nursing care and multi-disciplinary treatment of care services. This includes diagnostic and problem solving, demonstrating critical thinking skills, ability to prioritize and evaluate patients and their response to treatments, and interventions, and preparing nursing notes accurately documenting patient responses/outcomes to the plan of care, interventions, and expected outcomes at discharge.

Utilize performance improvement processes to address issues that arise related to patient care, appropriateness of care, and use of technology and efficiencies. This includes identifying and resolving patient care, unit and/or organizational issues in a timely manner and documenting appropriately.

Establish and maintain positive interpersonal relationships and promotes effective teamwork with patients, families, customers and the community, including demonstrating a professional, caring, and service-oriented attitude in all interactions, willingly collaborating and assisting all members of the health care team, and actively participating in relevant meetings, rounds, and/or teams as assigned.

Provide patient and family education based on readiness and the ability to learn, priorities of the patient’s multi-disciplinary needs, utilization of appropriate resources and expected outcomes at discharge.

Maintain privacy, security, and confidentiality of patient, staff and organizational data and/or information.

Use leadership skills to serve as a role model and enhance the delivery of care and services and the overall operations of the work environment. This includes using sound judgement in prioritizing and meeting patient care needs, supporting and following-up with patients, leading quality and process improvement initiatives, set a positive example to staff, and adhering to established policies and procedures.

Assist the Nursing Manager in directing the day-to-day functions of nursing activities.

Participate in the QA Process.

Admit, transfer and discharge patients as required. Complete accident/incident reports as necessary. Assist in discharge planning.

Perform administrative duties such as completing medical forms, reports, charting, etc. as necessary. Ensures that all pertinent information about the patient is charted correctly. Orders medication and supplies for patients and requisitions supplies.

Assemble charts and submit to business office and/or medical records department.

Take verbal orders from physician and transcribe to physician order sheets.

Notify physician and Nurse Manager if a patient is discharged without physician approval.

Administer medication and treatments as ordered by attending physicians. Maintain inventory of patient drugs and medication on the unit. Review physician orders to ensure accuracy and notifies physician of automatic stop order on specified medications.

Assist patients in getting ready for appointments and assist in arranging for transportation.

Perform tests and other diagnostic work as ordered by the physician. Take and record vital signs as required. Gives treatments and performs other professional services as ordered or required.

Provide restorative and rehabilitation programs for patients.

Monitor supply use and utilize supplies efficiently to minimize waste and maintain equipment used by nursing staff and reports and equipment not working properly to the appropriate responsible party.

Assists with coverage of staff shortages.

May perform minor housekeeping duties in assigned unit.

Review post-mortem preparation of expired patients, ensuring cleanliness.

Periodically checks food trays to ensure that the patient is receiving prescribed diet and recording fluid intake as directed. Assist in feeding patients as necessary.

Maintains noise levels in the facility at all times. May be required to assist aides and orderlies.

Instructs patients in self-help and self-care.

Ensure patient dignity and respect patients’ values at all times.

Operate within ethical standards. Follows established policies and procedures.

STAFF DEVELOPMENT

Attend and participate in scheduled training and educational classes to maintain licensure as a registered nurse.

Participate in developing, maintaining, and updating written policies and procedures that govern the day-to-day functions of nursing in your assigned unit.

SAFETY

Familiarity with possible exposure to blood/body fluids.

Follows established safety precautions, such as keeping floor dry and reporting spill immediately. Promptly reports any repairs needed by the maintenance staff.

Report hazardous conditions and safety violations.

PHYSICAL AND SENSORY REQUIREMENTS

Must be able to move intermittently throughout the workday.

Must be able to speak and write English language in an understandable manner.

Must be able to cope with the mental and emotional stress of the position.

Must be able to manage anger/fear/hostility/violence of others appropriately.

Must be able to see and hear or use prosthetics that will enable these senses.

Must function independently and have flexibility along with personal integrity. Ability to work effectively with patients, families, personnel, and other support agencies.

Must meet general health requirements, which includes a health evaluation form.

Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such a weight a minimum distance of 50 feet.

Must be able physically assist patients including lifting, moving, assisting and repositioning patients up to 50 pounds occasionally, including situations where a patient may be falling or need immediate assistance.

May occasionally have to lift/move up to 100 pounds.

May be necessary to assist in the evacuation of patients during emergency situations.

Must be able to handle emergencies, give emergency care and give emergency instructions.
