Licensed Clinical Social Worker- Behavioral Health Manager Job at ChartSpan

The behavioral health care manager is a core member of the collaborative care team, including the patient’s behavioral health medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients within ChartSpan’s Behavioral Health Program.

Duties and Responsibilities

● Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.

● Facilitate patient engagement within ChartSpan’s behavioral health program and follow-up care.

● Create patient education materials about common mental health and substance abuse disorders and the available treatment options.

● Participate in leadership meetings while taking the lead on developments and changes for ChartSpan’s behavioral health program

● Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.

● Facilitate referrals for clinically indicated services outside of the organization in coordination with the treating provider (i.e. social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

● Monitor and track patient follow-up and clinical outcomes within ChartSpan’s EHR.

● Assist nursing staff with developing care plans and assessments, as well as coordination follow-up treatment plans with the treating provider.

● Create proper documentation protocols and assist with compliance.

● Create emergency protocols

Requirements

● Master’s degree degree in Social Work

● Current LCSW license (COMPACT license preferred)

● 3+ years of experience

● Knowledgeable in CMS and regulations as it pertains to BHI

● Experience with screening for common mental health and/or substance abuse disorders

● Experience with assessment and treatment planning for common mental health and/or substance use disorders.

● Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.

● Experience with evidence-based counseling techniques

● Ability to lead and provide program development and guidance

Job Type: Full-time

Pay: $75,000.00 per year
