Description:

The PACE Center Lead Nurse is a PACE Center Primary Nurse responsible for the general oversight, assessment, planning and construction of the nursing care delivered to the participant, delivery and documentation of quality patient care in accordance with the departmental policies and procedures and in collaboration with other members of the Interdisciplinary Team. The PACE Center Lead Nurse has the additional responsibilities of working closely with the PACE Center Nurse Manager to ensure the highest standards of nursing practice, workflow efficiency and excellent participant outcomes. The PACE Center Lead Nurse serves as a point person for coordination of clinic activity and works closely with the Clinic Coordinator, Primary Care, Social Work and Home Care. The Lead Nurse also participates in the supervision of the Medical Assistants and Pharmacy Technicians.

$10,000 Sign-on Bonus

Schedule: Monday-Friday 8am-5pm

EDUCATION:

  • Baccalaureate degree in nursing required
  • Current Massachusetts license in nursing
  • Current BLS certification required

EXPERIENCE:

  • Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
  • A minimum of one year experience working with a frail or elderly population preferred.
  • Prior experience in interdisciplinary team setting a plus.

SKILLS/ABILITIES:

  • Basic physical assessment and triage skills.
  • Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats.
  • Ability to communicate effectively with professional and non-professional staff, outside providers, participants and families in person and by phone. Bilingual a plus.
  • Excellent customer service skills.
  • Flexibility, initiative, creativity and excellent organizational skills.
  • Basic computer skills including experience with word processing and email. Must be able to efficiently and effectively navigate windows-based programs and EBNHC’s EMR.
