Primary Care Clinic Integrated Behavioral Health Clinician (LCSW):

Looking for a dynamic LCSW to provide Behavioral Health services into a primary care clinic using the Behavioral Health Integration (BHI) model of care. Below are our objectives and key results for this position. This person can be fully in-person, fully virtual, or a hybrid of the two.

Success Factor 1: Within 12 months, become an integral part of 1 or more PCP teams.

• Adhere to the integrated behavioral health model into Primary Care Practice.
• Treat and provide brief interventions of primary care patients.
• Create relationship and build trust of provider, patient, and team.
• Exhibit effective use of Electronic Medical Records.

Success Factor 2: Within 18 months, create strong community partnerships.

• Develop collaborative network with community partners for specialized care.
• Develop process for coordination of external services.
• Coordinate continuity of Behavioral Health care with referrals.

Success Factor 3: Serve as subject matter expert for medical practitioners.

• Educate providers and team on importance of overall Behavioral Health of patients.
• Communicate modifications to Behavioral Health model with providers, team, and IPA based on workflow processes.

Education/Licensure Requirements:

• Master's degree required
• Successful candidate must have a current LCSW, and be willing to obtain Oregon licensure if current licensure is from outside Oregon

Preferred work experience:

• Previous Integrated BHC experience in primary care work environment
• At least one-year work experience as an LCSW
• Experience using Electronic Medical Records - Epic preferred

Salary and benefits:

• Competitive salary: $68,630 - $88,000 annually DOE
• Benefits offered: Medical and dental package; Life Insurance, Long- and Short-term disability, and AD&D; 401(k); and generous earned/personal time off package.
