Our Home Care Agency is seeking a Spanish-speaking Part-Time Intake Coordinator to join our team. The Intake Coordinator is the first point of contact for families, doctors, and healthcare facilities inquiring about the agency. The successful candidate will be responsible for obtaining and securing appointments for patient referrals and coordinating the necessary functions to bring the referral to conclusion.
Responsibilities:
- Establish themselves as a point of contact for MLTC coordination staff to assist with prompt placement of unassigned and/or difficult to service members.
- Maintain accurate records of patient information and referral sources.
- Communicate with referral sources to obtain necessary information.
- Coordinate with internal departments to ensure timely processing of referrals.
- Participate in agency quality improvement activities and staff meetings.
- Perform other duties as assigned.
Qualifications:
- Fluent in Spanish
- Must be able to work at least 3 days a week
- Strong communication and customer service skills
- Ability to work independently and as part of a team
- Excellent attention to detail and organizational skills
- Compassionate and empathetic towards patients and their families.
- Proficient in Microsoft Office and comfortable with technology
Job Type: Part-time
Pay: $20.00 - $23.00 per hour
