Seeking a full time Quality Assurance Nurse. Must be Licensed Vocation Nurse or Register Nurse.

Qualifications

• Graduate of an accredited school of nursing or school of vocational/practical nursing.
• Current license within the California as a LVN/LPN.
• At least two years of varied work experience as a licensed nurse and preferably one of the two years as QAPI or "QA" nurse
• Applicant must have excellent computer skills, ability to use HospiceMD
• Must be familiar with Medicare, Medi-Cal and other guidelines and regulations

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Performs chart and note reviews daily, upon recertification, at discharge, and as required.
• Works with the DPCS or Administrator to provide an on-going, comprehensive assessment of the quality of care and services provided.
• Coordinates all QAPI audits and studies as indicated in the plan.
• Develops and implements problem focused audits. Utilizes problem-solving techniques to address areas of non-compliance.
• Reviews all incidents, concerns, and grievances to ensure appropriate follow-up and documentation have occurred.
• Identifies patterns and trends and reports them to the DPCS or Administrator and QAPI committee.
• Conducts quarterly QAPI meetings, reports findings and coordinates development of action plans.
• Completes monthly reports to the Administrator and DPCS.
• Maintains current information and requirements related to applicable laws, regulations and standards.
• Assures the program is reviewed at least annually. Included is an evaluation of all services that were provided and patients/caregivers response to those services. Attention is given to: symptom management, pain control, stress management, continuity of care and inpatient care.
• Attends and participates in in-services, staff meetings and case conferences.
• Completes all documentation in an accurate and legible manner.
• Complies with hospice policies and procedures, laws and regulations and standards of practice
