As a Registered Nurse, you will be an essential part of the interdisciplinary team that makes a difference in the lives of our patients every day. You will perform a variety of specialized and highly skilled nursing services, coordinate the treatment plan with the other team members, and communicate with the patient’s physician and family. Care is provided one-on-one within the patient’s home.

Primary Responsibilities:

· Plan of Care: Perform case assessments and participate in the development of patient care plans. Manage the interdisciplinary care as applicable while promoting continuity of care.

· Supervision: Oversee the care of patients provided by Home Health Aides and Licensed Practical Nurses.

· Education: Provide education to the patient and family on disease process.

· Communication: Effectively communicates with the interdisciplinary team, patient and family regarding all aspects of care.

· Documentation: Complete all clinical documentation accurately and timely to ensure optimal care is provided.

· On-Call Rotation: Participate in an on-call rotation.
