Job Title: Healthcare Quality and Risk Coordinator

Position Summary:

The Healthcare Quality and Risk Coordinator is responsible for conducting clinical risk incident follow-up and investigation. Under the supervision of the Corporate Compliance and Privacy Officer, the Healthcare Quality and Risk Coordinator will partner with the organization’s Chief Medical Officer, Director of Nursing, Administrative and clinical system to ensure adverse and clinical-related near-miss incidents are thoroughly investigated. The Healthcare Quality and Risk Coordinator will also be responsible for identifying clinical risk trends and providing mitigation recommendations and strategies.

Principal Accountabilities/Responsibilities:

Monitor incident reporting system for patient harm events and unanticipated outcomes by analyzing medical records, identifying potential exposures, and escalating risk reduction methods.

Triage and investigate incidents by collecting medical documentation from sites to analyze and escalate accordingly.

Timely document incident and investigative follow-up and mitigation notes into Cooperative Health’s incident reporting system.

Perform special projects that involve data gathering, preparation of reports, trend management, and escalation.

Collaborate with staff to identify, monitor, and report on areas of risk and litigation trends.

Report significant/high-risk areas to necessary departments.

Prepare risk findings for CQI committee so that CQI can consider and enact clinical risk recommendations to address quality issues and mitigate further legal risk exposure.

Provides weekly data reports to Corporate Compliance and Privacy Officer.

Serve as a patient, employee, and organizational advocate when incidents occur.

Performs other duties as assigned.

Qualifications:

Medical Assistant Certification with 3 years of proven administrative experience at minimum.

Graduate of an accredited school of nursing, with an Associate’s degree, preferred.

Current state license as a registered nurse or licensed practical nurse in the state of South Carolina.

Must have minimum of four (3) years’ experience in a clinical practice setting.

Certification in risk management and or clinical quality improvement preferred.

Previous Athena EMR usage preferred.

Previous Joint Commission (JAHCO) and or infection control experience preferred.

Excellent interpersonal communication and problem-solving skills.

Excellent written and oral communication skills with particular emphasis on presentation skills.

Knowledge of statistics, data collection, analysis, and data presentation.

Must be able and willing to drive to all four counties (Fairfield, Lexington, Newberry, Richland) covered by Cooperative Health.

Must have reliable transportation.
