POSITION OVERVIEW*

HomeRise seeks an experienced social service professional to conduct programming and offer resident support to single adults formerly experiencing homelessness in San Francisco. Supervised by the Resident Services Manager, the Health Navigator is responsible for guiding residents through and around barriers in the complex healthcare system. The Health Navigator will provide support to overcome obstacles faced in accessing or receiving services needed to maintain well-being, independence, and self-sufficiency. This role will provide practical support services with a strong emphasis on purposeful case management, goal achievement, and groups to single adults within permanent supportive housing in collaboration with a multi-disciplinary site-based team comprised of Resident Service Team Leads, Resident Service Counselors, and DPH nursing staff.
All services are provided within the context of Housing First and Harm Reduction.

ESSENTIAL FUNCTIONS

Engage residents in compassionate ways to build motivation towards health initiatives in their own lives

  • Conducts assessments, provide case management/health navigation services for up to ten residents, create and track service plan goals, and ensure that appropriate follow-up and/or referrals are complete and documented.
  • Identifies barriers which interfere with resident’s ability to access healthcare, in conjunction with site-based service team.
  • Develops linkages with and coordinate access to healthcare facilities (clinics, hospitals, specialists, etc.)
  • Promotes resident’s involvement with the healthcare system through collaborative scheduling and tracking of appointments, providing transportation, and post-appointment follow-ups. Provides accompaniment, when necessary, to act as an intermediary for dialogue between resident and healthcare providers.
  • Advocates for resident’s right for fair treatment and access to healthcare.
  • Provides health education and support in accordance with harm reduction principles.
  • Verbally deescalates tense, high pressure, and emotionally charged situations.
  • Creates and implements health-based education and resource groups for residents and staff.
  • Consults with on-site multi-disciplinary staff to support health navigation for residents not directly working with this position and/or with engaging in health and case management services provided on site.
  • Complete documentation per agency policy.
  • Participate in meetings and organizational initiatives.
  • Other duties as assigned.

  • Status: Full-Time / Non-Exempt
  • Pay Rate: $27.00/ hour
  • Schedule: Monday through Friday 9 AM – 5 PM
  • Union: This position is under the collective bargaining agreement with SEIU 1021
  • Reports to: Resident Services Manager

QUALIFICATIONS

*
Minimum of two years’ experience working, in paid and/or volunteer roles, successfully with issues of substance use, mental health, criminal background, chronic health issues, systemic racism, poverty and other potential barriers to economic self-sufficiency.

  • Proficiency with supportive housing, housing first, trauma-informed, motivational interviewing and harm reduction principles.
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.
  • Knowledgeable or willing to learn general medical terminology.
  • Familiarity with local SF and Bay Area community resources, health resources, and service programs.
  • Excellent computer skills (Outlook, MS Word, Excel, Salesforce or other cloud-based database).
  • Strong interest in doing social justice advocacy.
  • Desired qualification: Fluency in Spanish/Mandarin/Cantonese/Russian a plus.
  • Valid phone number required.
