Patient Care Services Coordinator

The Patient Care Coordinator is an experience Registered Nurse who displays excellent leadership, communication and analytical/problem solving skills. They are responsible for assisting the nursing staff as needed with clinical processes, work flow and as well as providing consultative support for patient care. The PCC provides for the coordination of rehabilitation nursing care to patients in a specifice unit/program. This role functions as a clinical rehabilitation nursing resource to all nursing staff and interdisciplinary team members. The PCC coordinates the overall education plan for patients and assists the RN assigned to the patient in providing educational resources, consulation and guidance. In conjuction with direct care providers, the PCC provides guidance and consultaion to designated preceptos on the unit and assists the nurse manager in coordinating care planning, assessing the patient experience and participation in various clinical inter-disciplinary work groups including telehealth, skin care, fall prevention and other dashboard outcome measures. Additionally, in collaboration with the nurse manager, the PCC will provide direct care for a selected number of patients as needed.

Experience:

  • Must be a Registered Nurse licensed in Pennsylvania
  • BSN and CCRN required
  • MSN preferred
  • Minimum 5 years of licensed professional nursing experience and minimum of 3 years in rehabilitation nursing.

Responsibilities:

  • Within the role of patient care, the PCC will assess health status, illness conditions, response to illness, and health risks of individuals, families, and groups, using advanced skills in history’ taking , physical examination as needed and analysis of objective findings. Assesses resources, strengths and weakness, coping behaviors, and the environment.
  • Within the role of patient care, the PCC diagnoses human responses to actual or potential health problems and continually seeks validation of the etiologies of patient care problems. A key to success in diagnosis is the ability to examine alternative possibilities as to the nature of a health problem. Confidence and accuracy in judging which diagnostic hypotheses are sufficiently supported are enhanced by the reasoning ability of the PCC.
  • Plans for therapeutic nursing intervention in collaboration with the patient and the interdisciplinary team. The goal is to enhance the problem-solving and self-care abilities of the client whenever and to whatever extent possible. The PCC works with other team members and external providers to maximize resources available to the client and family.
  • Intervenes to assist patients, encouraging patients to participate in their own care as much as possible. Interventions may include, but not limited to direct nursing care, assuring that patient education is completed, direct teaching, and consultation with or referral to other health care providers. The NCCC will take responsibility for coordination of care that involves other health professionals or resources, thereby providing continuity and helping the patients deal effectively with the health care system once discharged. They serve as a “navigator or bridge” between inpatient and outpatient services.
  • Evaluates outcomes of care with the inter-disciplinary team by measuring intervention effectiveness with the team; implementing team recommendations with the patient and family as guided by each rehab program. A critical evaluation of the success of interventions is carried out with adjustments being made as needed.
  • Assists the nurse manager as needed in assessing the patient experience by rounding on patients. Gathers information and works in collaboration with the nursing management team to influence positive patient experiences.
  • Documents care, identifying progress toward outcomes, intervening to overcome deviations in the expected plan of care, reviewing the nursing care plan with patients in conjunction with direct care providers, interacting with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults, reporting personnel and performance issues to the nurse manager, maintaining on going communication with the UR and Case Management staff regarding variances from the care plan or transfer/discharge plan.
  • Resolves issues that could affect smooth nursing care progression, fostering peer support, providing education to others.
  • Mentors and coaches all preceptor/RN staff to assure that specific rehabilitation nursing education core competencies are completed and applied consistently and safety.
  • Complies with hospital and legal requirements by fostering nursing practices that adhere to the hospitals’ and nursing’s philosophy. Goals, and standards of care; requiring adherence to nurse practice and other governing regulations.
  • Respects patients by recognizing their rights; maintaining confidentiality.
  • Maintains quality service by establishing and enforcing organizational standards.
  • Maintains patient care database by entering new information as it becomes available; verifying findings and reports; backing up data.
  • Maintains professional and technical knowledge, attending educational workshops; reviewing professional publications, establishing personal networks, benchmarking state-of-the-art and evidence-based practices; participates in professional organizations.
  • Contributes to team effort by accomplishing related results as needed through active participation in required inter-disciplinary meetings.
