JOB TITLE: Experienced Family Nurse Practitioner

Job Summary:

Healthy Families, a division of Georgia Mountains Health Medical Group (GMH), located in Chatsworth currently has an opening for a full time, experienced Family Nurse Practitioner with a minimum of five (5) years chronic experience. GMH is a non-profit, Federally Qualified Health Center (FQHC) mission driven organization, committed to providing compassionate and quality to care to families in North Georgia. Chatsworth, Georgia is a quaint small town away from the busy city, close to the mountains where peace, tranquility and picturesque scenery can be found. You can live and work in a diverse and family-friendly environment and enjoy a low cost of living while practicing in this beautiful small town that is accessible to Atlanta area or Chattanooga in less than one hour.

The Nurse Practitioner practices in collaboration with physicians and other healthcare staff to provide care management and care coordination. Provides direct patient care for men, women and children. Demonstrates ability to identify and document the needs of complex patients, identify, diagnose, and document chronic disease and perform comprehensive exams. Establishes a practice of using UDS measures for quality health care improvement. Works with GMH established nurse practitioner workflow schedule of 18-20 patients per day. Potential to earn up to or more than $4,000 per month in bonus incentive.

Requirements:

Qualified candidates must be Board Certified Family Nurse Practitioners with a valid/active Georgia Nurse Practitioner license.
