Position will serve as a contact point between the SafeCare Program and possible referrals in the community; and will be responsible in educating community partners and potential consumers about the SafeCare Program. This position will develop on-going, mutually beneficial relationships with referral agencies and community organizations through frequent personal contact. This position will also provide home based clinical and social services to high risk families referred to SafeCare. The goal of the services is to reduce the risk of families for abuse and neglect of their children.

Essential job functions include, but are not limited to:

  • Facilitates outreach opportunities within the community by scheduling presentations that will inform community partners of the services that the SafeCare Program offers.
  • Demonstrate a thorough and complete knowledge of the program including our vision, mission and values; the services we provide; and how we differentiate ourselves from other home visitation programs.
  • Identify, evaluate, and prioritize potential referral sources within the programs demographics.
  • Establish and maintain professional relationships with all referral sources, including but not limited to the following: hospitals, non-profit agencies, home visitation programs, churches, libraries and police departments.
  • This position will divide time between facilitating outreach and serving families by providing the SafeCare model to high risk families in the home environment. Providers will work around the families schedule and provide services, which are sensitive with respect to family culture.
  • Utilize case management to advocate for and assists families with community resources and accompanying the families if necessary to obtain needed services. Assists the family with crisis stabilization and arrange for any follow-up services needed after case termination.
  • Facilitates families in the development of special skills or increased knowledge in the areas of child health, home safety, parent-child interaction, behavior management, child development, advocacy, and communication and negotiation skills.
  • Provides Motivational Interviewing and problem solving in the areas of substance abuse, domestic violence and mental health (other areas as identified by family).
  • Providers will meet fidelity to the SafeCare model within nine months of employment. Providers will have ongoing consultants shadow them up to four times per month to ensure fidelity to model.
  • Maintains clinical records in a timely manner as required by the program, agency, state and federal funding sources and accreditation authorities. Completes all required SafeCare forms and collect data for evaluations and research of the program effectiveness.
  • Providers will conduct themselves in a professional manner while developing and maintain positive relationships with consumers, community partners and with consultants.
  • Participates in trainings/workshops related to SafeCare model and in expanding knowledge base in domestic violence, substance abuse, mental health and motivational interviewing.
  • Must maintain an accurate weekly myAvatar calendar to ensure safety. Must return phone calls and pages promptly. Must check and respond to emails daily.
  • Abides by agency policy and program procedures at all times.
  • Performs other duties as required and/or assigned by supervisor and/or executive staff.

POSITION REQUIREMENTS:

Position requires a Bachelor’s Degree in Marketing, Public Relations, Social Work or closely related field. Must display proficiency in the MS Word processing software and intermediate general computer skills. Excellent written and oral skills are essential, along with strong organizational skills. Must demonstrate good interpersonal skills and the ability to effectively communicate verbally and in writing with English speaking consumers. Must be able to bend, stoop and move about facilities freely, climb stairs, lift up to 30 pounds and be stationary for extended periods. Casual professional dress is required. Local travel required.


Must possess and provide verification of valid Oklahoma driver’s license during employment with NorthCare. Must maintain and provide verification of current automobile liability insurance including uninsured motorist coverage. Must have and/or complete Core Level Co-Occurring Training and Trauma Informed Training within 60 days of employment with NorthCare. Must have TB test within 12 months prior to employment with NorthCare, or obtain within 30 days of employment with NorthCare. Must possess and maintain current certification in CPR, First Aid and CAPE certifications. If not current upon hire, must obtain CPR, First Aid and Therapeutic Options certifications within 30 days of employment with NorthCare.
