Job Description:

An exciting opportunity leading emergency department care management is available to candidates invested in supporting caregivers who provide fast paced, intensive, patient centered care. This position leads emergency department care management at Utah Valley, American Fork, Alta View, and Riverton Hospitals. This position partners with Emergency Department managers, providers, and nurses as an interdisciplinary team. This team supports the needs of patients for trauma, mental health, substance use, protective services, care planning, and community referrals. Managerial duties include staffing, outcomes, staff development, and innovation with a high performing team of caregivers.

Position Details:

This role will manage operations of the Emergency Department Care Management teams and has coverage for escalated needs 24/7 in a rotational support with team leads.

General coverage for leadership meetings and duties are Monday-Friday from 0830-1700. This position is on-site at Utah Valley Hospital 3 days per week, with an additional two days at Alta View, Riverton, or American Fork Hospitals. There is some ability for Flexwork as needed to round and check in with staff after hours. The majority of this position is on-site and has some opportunities for virtual support.

Minimum Qualifications:

Master's Degree in Social Work. Education will be verified.

Current State of Utah LCSW license.

3 years of experience in hospital case management, social work, or related area.

Preferred Qualifications:

Le adership and strategic organizational experience.

Experience p roblem solving and coordinating with community agencies to mitigate barriers.

P rior ED care management or care management experience.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
