The Diabetes Health Educator (DHE) provide self-management support to patients living with diabetes in partnership with the Healthy Living with Diabetes team (3 other DHEs) and the Diabetes Registered Nurse who ensures clinical compliance and integrity. The DHE conducts one-on-one educational visits, collaborates with primary care providers to facilitate group medical visits, and provides home visits and mobile medicine in locations across Yolo County.


HOURS/SITE: Full-time, 40 hours per week, Monday – Friday, primarily at one CommuniCare site (West Sacramento) as well as home visits and Mobile Medicine visits within Yolo County. Travel with personal vehicle is required. Evening and weekend hours occur at times.


RESPONSIBILITIES:

  • Develop and case manage a panel of patients with diabetes and prediabetes and deliver evidence-based, patient-centered care and effectively provide diabetes self-management support.
  • Collaborate with patients, their assigned Primary Care Providers, the Diabetes Registered Nurse, and a Registered Dietitian to ensure continuity of care.
  • Work with the Quality Improvement team to support patients to improve quality measures related to diabetes management.
  • Conduct clinical activities (including retinal eye exams, monofilament foot exams, sample collection and analysis, continuous glucose meter application and education) according to standing orders for DHEs.
  • Work with Food Programs staff to support patient food access and food agency/self-efficacy.
  • Coordinate and provide home visits to patients who face barriers to transportation or may otherwise benefit from in-home education.
  • Coordinate and facilitate Group Medical Visits and education and support sessions for patients with diabetes in collaboration with a Primary Care Provider and other staff.
  • Collaborate with the Mobile Medicine team to integrate and deliver diabetes education with health care services provided to community members outside of the clinical setting (including migrant and seasonal agricultural workers and individuals experiencing housing insecurity)
  • Other duties as assigned

Education:

  • Bachelor’s degree in health and human service field and at least one year experience in a health clinic, counseling agency, or other closely related social service agency,
  • OR High School Diploma/GED and a minimum of 3 years working in a clinic or social service setting.
  • Diabetes Community Care Coordinator Certification preferred, and certification is required within 90 days of employment.


Experience:
Experience working with persons living with or at risk for diabetes and other chronic diseases. Experience providing individual health education and teaching/facilitating classes. Commitment to and lived experience or experience serving low-income, under/uninsured population. Experience working within a community health center or primary care setting.


Special Skills/Equipment:

  • Bilingual (Spanish and English) is highly preferred; other desirable language skills in addition to English include Russian and Punjabi
  • Ability to communicate effectively with patients and patients’ family members.
  • Experience working in electronic health records.

Ability to deal effectively with a diversity of individuals, externally and internally, at all organizational levels.

Specific vision abilities required by this job include close vision. Work is primarily performed in a typical office environment with moderate equipment noise. COMPLETED COVID-19 VACCINATION REQUIRED


SALARY/BENEFITS:

  • The pay range for this position is $25.70-$36.20. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
