Community Care is hiring Job Title to staff one of the convents we serve in the greater Milwaukee area. As a member of our team of compassionate caregivers, you will serve and care for a group of retired nuns living in a peaceful, tidy, and well-supplied residential setting.

Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. We coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. We are Wisconsin-based with local offices supporting each of the 15 counties we serve. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position located in Dane County.
Care Manager/Case Manager Job Responsibilities:
•
Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
•
Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
•
Certification: Eligibility for long-term care functional screener certification is required
•
Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
