The Crisis Case Manager/Critical Intervention Specialist works in a 2-person team to manage a caseload of clients that are referred to us by the Golden Gate Regional Center, covering throughout the Marin, San Francisco, and San Mateo counties and participates in weekly Clinical and administrative meetings at our South San Francisco office.

Our ideal candidate would be someone who is graduating or graduated from a Master's program for Social Work, Psychology, Counseling, Marriage and Family Therapy, or another related field. They would be a road warrior; who would have no issues driving in and around the respective coverage area and responding to crises. Our ideal candidate would have previous experience advocating, supporting, stabilizing, or working with those with either intellectual and/or developmental disabilities and their inner and outer circle. They would be adaptable - being comfortable with working independently the field, but ready to jump into a team at the office, in order to create a cohesive collaboration; bringing in your individual expertise to the forefront. The SF team might be small but they are mighty!

Case Management duties include, but are not limited to:

  • With a partner, visiting the client and collaborating with circle of support/collateral contacts at least once a week, and daily submission of case notes.
  • Periodically carry our crisis hotline rotation with the intent of de-escalating the situation and respond in-person when necessary.
  • Develop Case Reviews and create Stabilization Plans for your client.
  • Attend client meetings and Regional Center clinics as directed, as well as all workshops, trainings and in-services.
  • Attend and participate in clinical meetings with community resources (such as Psychiatrists, Social Workers, Physicians, Therapists, etc.).

What you will need to bring to the table:
Required

  • Master's Degree is required, preferably in Psychology, Counseling, Special Education, Social Work or must have a masters in a related field. Receipt of official copy of Master's Degree prior to first day of work or official transcripts that will be required no later than 30 days from start of employment.
  • Must have your own reliable means of transportation and valid CA Driver's License. Additionally, must be willing to commute to various settings to meet with clients' circle of support and the identified client (i.e., family homes, Adult Residential Facilities, day programs, schools, hospitals, etc.).
  • Proof of current and valid car insurance with minimum $300,000 bodily injury and property damage combined coverage, or $300,000 Bodily Injury and $100,000 property damage coverage.
  • Ability to work full-time, weekends and evenings as needed.
  • Must have a designated home office/workspace due to Covid-19 safety guidelines. Team meetings and client sessions may take place via Zoom.
  • Must have high-speed internet
  • Must have appropriate computer skills and be able to handle multiple simultaneous tasks.
  • Must demonstrate good interpersonal and communication skills, be able to maintain confidentiality, and present themselves professionally.

Preferred

  • One year experience in mental health providing counseling and/or working with persons with Autism and experience utilizing ABA strategies.
  • Previous crisis intervention and/or de-escalation.
  • Experience with Welligent or other EHR (electronic health record) database
  • Spanish, Cantonese, Mandarin, and/or Tagalog are STRONGLY encouraged to apply.
