Description

Provide assistance to community members and the public with accessing services and resources to improve health and safety. This is a part-time, benefit-earning position working an average of 20 hours per week with a set schedule (TBD based on candidate) within the 8:00 am - 4:30 pm workday. There may be occasional evening and/or weekend hours needed.

Essential Functions

  • Assist community members in finding and accessing medical and non-medical services and programs.
  • Follow up with community members after public safety encounters to provide information on available resources.
  • Conduct outreach to implement programs in the community that promote, maintain, and improve individual and community health in a culturally sensitive manner.
  • Provide social support and guidance.
  • Advocate for individuals and community health needs.
  • Evaluate data to help identify community health needs. Gather information from clients on issues such as mobility, transferring, access to food, prescription compliance, care giver needs, existing support systems, socio-economic challenges and access to health care.
  • Maintain confidentiality and ensure client personal information is protected.
  • Assure that necessary records are accurately kept and submitted.
  • Assist with scheduling and presenting educational programs to various segments of the community.
  • Create and locate documents, flyers, and brochures in a variety of languages as applicable to the community.
  • Perform other duties and responsibilities as needed, apparent, or assigned.

Qualifications

MINIMUM QUALIFICATIONS


  • High school diploma or equivalent
  • Valid Minnesota driver’s license
  • Ability to communicate effectively verbally and in writing to meet the essential requirements of the position with people from all cultures and backgrounds.
  • Ability to use various software applications to enhance the work of the department.
  • Ability to present in front of groups as well as interact effectively one-on-one.

DESIRED QUALIFICATIONS
  • Prior experience working in patient advocacy, long term care, community health, community risk reduction or community EMT/paramedicine.
  • A degree or coursework leading to a degree in Education, Community Health, or Social Work.
  • Fluency in a second language other than English.
  • MN Community Health Worker Certification
