The Community Liaison is a qualified person appointed by the Agency Administrator, who is responsible for developing new business for the Agency. They are also responsible for identifying community referral partners, educating them on our services, and developing relationships. The Community Liaison continually evaluates the timely adjustment of marketing strategies and plans, recommends changes in the marketing plan, and executes visits to businesses and/or individuals that would benefit from the services of the Agency.

ESSENTIAL FUNCTIONS

1) Develops new referral sources and strengthen existing relationships with existing referral sources.
2) Sell and promote agency services in the community via strong prospecting and by building strategic alliances.
3) Create and keep up to date a business plan within assigned area according to the company’s goals.
4) Makes in-person and telephone contact with referral sources. Sets appointments, facilitates meetings with referral sources and appropriate Agency personnel.
5) Maintain documentation in Sales Force relating to all sales activities.
6) Pre-plan weekly marketing activities.
7) Maximize efficiency and cost effectiveness in daily activities
8) Maintains up-to-date knowledge regarding the Medicare and Medicaid Anti-Kickback Act.
9) Prepares weekly reports documenting activities completed for the Agency in the community.
10) Represents the Agency at community and/or business functions; promotes existing and new Agency programs.
11) Works with clinicians in providing referral information that supports the development of an individual plan of care for the patient being referred to the Agency.
12) Assists Agency personnel in care coordination of patient services. Serves as a liaison between the Agency, referral sources, patients and clinicians.
13) Researches needs of physicians, patients to aid in program development within the Agency.
14) Stays up-dated with changes in OASIS and home health PPS requirements, and Medicare guidelines.
15) Other duties as requested by the Agency Administrator.
