General Functions:

This position is a part of an integrated care team. The primary goal of the Community Health Worker (CHW) is to increase client awareness and access to healthcare to improve the overall quality of life for women within and recently exited from homeless shelters (Ruth’s Place) and low-income women with children (Caring Alternatives). The CHW will work with clients from socioeconomically diverse backgrounds, in both the home and facility environments. The CHW will collaborate with MCO or health system, referral agencies, medical providers, primary care teams, and other agencies to improve overall client care, as well as outcomes. The CHW will bridge conversations between clients and identified community resources, removing barriers that prevent them from accessing health and social services. This position requires the ability to work within a team to provide timely, appropriate, female-centered care equitably. Duties include but are not limited to assessing SDoH needs with a focus on healthcare access and affordability, providing complex case management and care coordination, providing options-based counseling and health education, maintaining case files, developing relationships with area agencies and primary care physicians, reporting on client progress success, and communicating these to appropriate team members.

Scope of Duties:

  • Demonstrate knowledge and professionalism while interacting with clients, team members, partner agencies, and the greater community.
  • Meet with clients regularly in the setting where they live to evaluate needs, progress on goals, and provide referrals, navigation assistance, and interventions as necessary.
  • Develop a person-centered plan for clients within 15 days of intake and ensure clients are connected to a primary care physician within 30 days of intake. Provide care coordination of physical and behavioral health services to clients.
  • Advocate for clients, ensuring that they are connected and have wrap around community services assisting with applying for services, navigating social and healthcare resources.
  • Ensure client files, visit reports, and notes are comprehensive and remain current.
  • Attend and participate in staff meetings and follow directives accurately, and timely.
  • Utilize data collected to develop/interpret reports and keeping statistics.
  • Establish and develop relationships with community resources and healthcare/insurance providers to increase client referral options.
  • While meeting with clients, assess and track their progress to ensure their person-centered plan addresses all necessary health and social needs. Update clients’ individualized plans, as needed, based on ongoing assessments.
  • Facilitate communication between all supports including, but not limited to, case managers, providers, social service agencies, peers, and family in order to meet the individualized needs of clients.
  • Provide ongoing education to clients regarding program requirements and assist clients with following policies and guidelines to maintain program compliance.
  • Attend community-based meetings and provide reports when requested.
  • Act as representative for Volunteers of America of Pennsylvania by demonstrating a positive and professional image toward clients and agency representatives.
  • Clock in/out daily and ensure that correct working hours are submitted to the supervisor.
  • In situations where policy is questioned or in case of emergency, the supervisor is to be notified.
  • Perform other duties as needed.

ADA Essential Functions:

  • Regularly required to stand, walk, sit, use hands to handle or feel, reach with hands and arms, climb stairs, talk, hear, and smell.
  • Regularly lift and move up to 25 lbs.
  • Ability to see clearly at 20 inches or less.
  • Ability to identify and/or distinguish colors.
  • Ability to observe an area that can be seen up and down or to the left and right while remain fixed on a given point.

Credentials/Abilities:

  • The employee in this position must possess either a bachelor’s degree for Registered Nurses or a Master’s degree in a health-related field and 3-5 years combined experience in a similar social service setting (LMSW or RN preferred), as well as a willingness to obtain the Community Health Worker certification when available.
  • A driver’s license is required; the employee will travel daily to conduct visits in clients’ homes and may occasionally drive for meetings, community events, etc.
  • The employee must possess strong clinical skills and the ability to make immediate decisions when faced with a crisis or unusual situation.
  • The employee must have a working knowledge of computer software, specifically Microsoft Office; including Outlook, Excel, and Word, as well as effective written and verbal communication skills.
