Job Description:
This position is open to all qualified applicants. Avita Community Partners is seeking the right candidate to serve as a full-time Community Living Support (CLS) CNA for IDD Services. The goal of this position is to support individuals with developmental disabilities to become active and integrated citizens within their communities. This will be accomplished by helping the individual to identify their personal preferences and implementing a plan of support which honors these preferences.
Staff in this position will work in cooperation with clients, families, community members, support coordinators, and consultants as identified through the Person-Centered Plans of the Service process. Must be Certified Nursing Assistant within the state of Georgia, hold and minimum of a High School Diploma or General Education Degree, and demonstrate sufficient reading, writing and computer skills so as to be able to perform the duties.
Candidates must be able to work a flexible schedule of late afternoons, evenings and weekends.
Job Requirements:
Must have a driver's license valid in the State of Georgia. Motor vehicle insurance as required by Georgia Law, and a good driving record is required. Applicants must not have six (6) or more points against their driver's license or a suspended or revoked driver's license within the last five (5) years.
A Pre-Employment Background will be conducted. This includes, but is not limited to, any of the following: Criminal (Both State and Federal Finger Printing), Driving, Educational and Employment record verifications/inquiries.
