Monarch is looking for A CHHA to see patients throughout various areas of San Diego county

To excel in this environment, the following skill sets and job functions are necessary:

Patient Care

  • Provides comprehensive personal care and light housekeeping to home care patients under the supervision of an RN, PT, ST or OT.
  • Performs simple nursing procedures following instruction by the nurse.
  • Assists patients with range of motion or other prescribed exercises under the instruction of primary therapists.
  • Provides ongoing observation of a patient's condition.
  • Document care provided according to the Plan of Care, including procedure performed and patient's response and condition.
  • Reports changes in condition to primary RN, PT, ST or OT, or supervisor.

Time Management, Organization, and Adaptability

  • Prepares for and prioritizes work assignment, arrives on time, and completes work assignment within the assigned shift.
  • Notifies supervisor of any barriers to completing work assignment on time.
  • Completes required paperwork including daily notes, route sheets and mileage reports according to departmental guidelines and timelines.
  • Demonstrates initiative by checking with the supervisor for additional assignments, tasks, and work related projects..

Knowledge, Skills, and Abilities

  • Knowledge of medical terminology, common conditions treated in the home health health setting, basic tests, treatments and procedures.
  • Understanding of basic computer applications, documentation systems, using scheduling programs.
  • Ability to safely execute Home Health Aide skills.
  • Ability to enhance patient care by effectively communicating and functioning in a team environment.

The following skills are preferred:

  • One year experience in acute care, home health, or hospice.
  • Proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau

Required credentials will include:

  • At least 18 years of age.
  • California Home Health Aide (HHA) - CA Department of Public Health

California Certified Nurse Assistant (CNA) - CA Department of Public Health

  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association
  • Current Driver's License - CA Department of Motor Vehicles
  • Current CPR certificate.

Other Qualification Requirements

  • Reliable transportation and adequate personal vehicle insurance coverage.
  • Demonstrates clean driving record in accordance with CA DMV and requirements of Monarch Home Health Driver Guidelines.

Agency requirements, including but not limited to:

  • Maintains current CHHA certification.
  • Attends a minimum of 12 hours inservice training per year.
  • Maintains competency and skills through attendance and participation in agency in-services and meetings.
  • Attendance at mandatory/non-mandatory staff meetings per agency standards.
  • Body mechanics requirements
  • Disaster procedures, fire, safety, hazardous materials and infection control.

Job Types: PRN, Contract, Per diem

Pay: $16.30 - $19.50 per hour
