Immediate Full Time opportunity to work in 31 bed ICF in Commack

General Purpose of the Job

As primary advocate for assigned group of individuals, coordinates and supervises Interdisciplinary Treatment Team. Acts as primary liaison with day programs, clinicians, outside providers and families.

Essential Duties and Responsibilities include the following. Other duties may be assigned

  • See that goals and methodologies are developed by appropriate disciplines to meet the individual's needs as determined by the Team.
  • Review all Monthly/Interim/Annual Notes for the Team in order to ensure they are complete, submitted on a timely basis, and accurately reflect the data.
  • Coordinate the schedule and calendar for all Interim/Annual Conferences to ensure appropriate time frames are met. Call special IDT meetings as needed.
  • Attend and conduct case conferences as well as Interim/Annual Conferences.
  • Ensure that all disciplines provide ongoing and refresher in-services for direct care aides relative to new goals, as well as the review of already existing goals.
  • Track notes to ensure they are co-signed by appropriate clinical supervisor, i.e., developmental specialist, PT, speech, reflecting progress shown by data sheets.
  • Flexible work schedule as needed.
  • Coordinate expenditure of personal funds and clothing allowance as required.
  • Assure that all documentation and necessary information is filed in program charts in a timely fashion.
  • Knowledge of ICF regulations in order to ensure compliance.
  • Assumes other duties and activities as directed by Program Supervisor.


Education and/or Experience

Minimum requirement Bachelor's degree, Master's degree preferred. Must be eligible for QIDP status Experience with the developmentally delayed and disabled preferred.

NYS Drivers License required, driving for at least 1 year.
