Each lead case manager employee at There Is Hope shall be responsible for the performance of assigned staff. The lead case manager is responsible for assisting and supporting the administrator. The lead case manager is a key member of the team, responsible for the supervision of up to six (6) case manager.

Requirements:

1. Documentation of the required education and experience requirements shall be maintained in the personnel file.

2. Employee mush undergo a pre-employment physical examination. This examination will be given at the employees expense by a licensed physician and will include a TB test as requires by Community Care Licensing.

3. Employee mush submit to a live scan fingerprints process. Each employee must receive clearance from Department of Justice, Federal Bureau of Investigation and Child abuse Index per community care licensing.

4. Employee mush sign a statement acknowledging the child and elder abuse and neglect reporting.

5. Participate in employee orientations and ongoing training.

6. Have current California Driver License and automobile insurance.

7. Employee mush have the ability to use office machines, word processors, computers, etc. to prepare the necessary documentation, reports, etc. regarding the clients being served by There Is Hope FFA, responsibilities in Penal Code Section 11164 et seq.

8. Employee shall complete and minimum of one-hour training in the area of child and elder abuse recognition, identification, assessing and reporting.

9. Attainment of a least 21 years of age.

Responsibilities and Obligations:

1. Orientation and training of new social work personnel

2. Review and oversight of social work personnel casework

3. Ensure that assigned social work personnel perform their duties in compliance with applicable laws, regulations, policies and procedures

4. Responsible for the supervision for up to six (6) case managers

5. Assist the administrator in the daily operations of administrating the office

6. Provide appropriate and effective directions to case manager, encourage innovation while motivating to accomplish goals and build teamwork

7. Directly responsible for completing employee evaluations for assigned case manager

8. Be committed in demonstrating personal, organizational integrity an character

9. Participating in planning and development of enhancement programs geared to upgrading family foster care.

10. Review foster family care evaluation and placement recommendations

11. Communicate and interact fully and successfully with the case manager, adoptive parents, community agency workers and There Is Hope FFA management

12. Participate in There Is Hope FFA events

13. Be accessible and maintain an visible presence with the certified families

14. Attend community meetings and county events

15. Promote There Is Hope FFA at local, state and national level

16. Maintain a visible presence with families

17. Maintain individual records directly related to the performance of the case manger under his/her supervision

18. Schedule private meetings with each social worker for he purpose of reviewing and discussing cases

19. Make themself available to the case manager who seek either guidance or other assistance in developing treatment plans or preparing reports

20. Make recommendation for rewarding/praising social workers performance based on monthly evaluations

21. Directly responsible for quality assurance for current foster child's files, Review on a monthly basis the files for all children placed under his or her responsibility, meets with case manager assigned to the case an correct deficiencies (if any)

22. Ensure that all reports are submitted b y case manager in a timely manner

23. Responsible for the collection of competed reports for the discharged/reunified minors.

24. Schedule and conduct home evaluations

25. Assess thoroughly and respond in a timely manner to all treatment issues impacting children and families

26. Participate in the orientation and training of new case managers

27. Monitor the evaluation and assessment of the application of the potential certified family homes

28. Formulate, compose, edit and/or review reports, letters or necessary forms

29. Monitor Special Incident Reports to assure child safety and title 22 compliance

30. Assess and report in a timely manner t he details and circumstances concerning any allegations made by the youth

31. Be knowledgeable and able to effectively apply There Is Hope FFA policies and procedures along with Federal, State and county regulations

32. Adhere to Child abuse and elder abuse reporting requirements

33. Develop and provide trainings for Community Care Licensing recommended plan of corrections with respect to investigations outcome or Special Incident reports

34. Participate in the overall Accreditation/quality improvement process

35. Liaison between the department, county case manager and There Is Hope FFA to resolve and/or misinterpretations between said entity's personnel

36. Complete projects in a timely manner

37. Have time management skills

38. Remain accessible by cellphone 24 hours/7 days

39. Perform other duties as requested.
