Case Manager coordinates the service needs of individuals with intellectual disabilities or other related disabilities as they move in and out of various programs. This position monitors progress and actively participates in planning for each individual in his/her caseload to include initial and annual Assessments and Support Plans, completes monitorships monthly with each person in caseload, maintains electronic case notes to document Case Management activities to support Medicaid billing, provides crisis intervention as needed, coordinates services with miltiple agencies to meet the needs of individuals, etc. Applicant must possess a bachelor's or graduate degree from an accredited college or university with ability to provide an official transcript and at least one (1) year of experience working directly with persons with an intellectual disability or a related disability; SC State (PEBA) Retirement and Health Benefits

Job Type: Full-time

Pay: $15.00 - $19.00 per hour
