To be successful in this role, you will be responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards.

Job Requirements

  • Social Worker licensed in the state of California
  • Recent acute care experience
  • Completing Social Work Assessments, providing resource information, end of life discussion or support, assessing suspected neglect or abuse cases, active member of the health care team working with the patient, family and providers.
  • Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
  • Care Coordination by demonstrating efficient throughput while assuring care is sequenced and at appropriate level of care

Job Duties:

  • Plans and coordinates post acute services and needs.
  • Coordinates the discharge planning function.
  • Meets with patients and families early on admission to complete the discharge planning assessment to develop the plan for the discharge.
  • Actively participates in High Performance Interdisciplinary Rounds daily
