Essential Functions

Reasonable accommodations may be made to enable an individual with disabilities to perform the essential functions. Provide Adult Mental Health Targeted Case Management which includes

  • Conduct Client interviews via telephone, home visits, and office visits to determine need for services and financial assistance; advise of possible resources to meet needs.
  • Complete functional assessment, Individual Community Support Plan (ICSP) within 30 days and every 180 days following. Monitoring diagnostic assessment requirements needed for ongoing case management services and other services.
  • Provide clients access to services and community resources; attend outside agency staffing with clients and care teams; provide or coordinate transportation; advocate for clients and families.
  • Intervene in crisis situations; coordinate with law enforcement, medical personnel, and other community agencies to protect individual and community safety.
  • Maintain case records that meet State and County guidelines; prepare regular correspondence and special reports including petitions for commitment.
  • Consult with other medical, psychological and legal professionals in conjunction with case objectives; prepare for court appearances; provide legal testimony as needed.
  • Prepares information and recommendations with court cases and proceeding, consults with attorneys, interprets and implements judicial findings, and assists the court in determining the need for commitment by screening persons to determine their need for treatment as MI, CD, or MI&D.

Minimum Qualifications

Bachelor's degree in social work, psychology, Human services, or related field and moderate experience with mental health, social services and/or chemical dependency. Knowledge of resources and services for mental health/chemical dependency. Knowledge of Rule 79 case management, crisis de-escalation preferred.

Special Qualifications

2,000 hours of supervised experience in the delivery of mental health services

Required Knowledge, Skills, and Abilities

Knowledge of the principles, techniques, and practices of social work.

Knowledge of the principles of mental and chemical health and the ability to identify psychological disorders of infancy, childhood, adolescence and adulthood.

Knowledge of medications commonly used to treat mental/emotional disturbance and the implications.

Ability to organize work effectively.

Knowledge of Federal and State Laws and programs relevant to human service programs.

Knowledge of social and economic problems.

Knowledge of human behavior and psychological implications.

Ability to communicate effectively, both orally and in writing.

Knowledge of the operations, procedures, and services of the County, the schools, community resources, and the functions of other social service and financial agencies.

Knowledge of the current Federal and State laws relevant to human service programs and an understanding of the State Human Services programs and the County’s relationship to these programs.

Ability to adapt and change to meet complex needs and law changes.

Ability to establish and maintain effective working relationships with co-workers, clients, and the community in general.

Ability to adhere to and promote the agency mission statement, values and goals.

Regular and timely work attendance.

Ability to work independently, manage time productively, and prioritize case loads

Ability to plan, select, and utilize a variety of appropriate residential and other treatment facilities and psycho-social interventions, including individual and group treatment techniques and processes, to provide the opportunity for behavioral change and psychological/emotional growth.

Ability to exercise judgement in making decisions.

Work occasionally requires exposure to fumes or airborne particles, exposure to toxic or caustic chemicals, exposure to outdoor weather conditions and exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment; work is generally in a moderately noisy location (e.g. business office, light traffic).
