Navigation and Information Services (Office of Multi-Cultural Services) within the Initial Contact and Assessment area of the Human Services Public Health Department is seeking a bilingual Case Management Assistant who is fluent in Spanish and English, to be part of a team that assists Hennepin County residents in accessing services, and that supports their needs to allow them to live healthy, safe, stable, and independent lives.

Location and hours:

This position will be on-site at the South Minneapolis Human Service Center at 2215 East Lake Street, Minneapolis MN 55407. This position will requires staff to work at other sites as needed. This position will also participate in community events with the possibility of some remote work in the future. Work hours are typically 8 a.m. – 4:30 p.m., Monday through Friday with occasional evening and weekends hours (not to exceed 40 hours per week).

In this position, you will:

  • Seek out, interview, and assist people in need of agency services with an emphasis on the Spanish speaking community in Hennepin County.
  • Serve as a cultural broker between county staff and members of the community.
  • Conduct informational presentations before neighborhood groups, schools, healthcare providers, non-profits, Mutual Assistance Associations, etc. to establish relationships with other resource providers.
  • Visit individuals and families in their homes (if necessary) to explain supportive services and resources available to help them meet their needs.
  • Collaborate with schools, medical personnel, and other agencies to facilitate communication between them and our mutual clients.
  • Inform county residents about available resources and assist them in completing applications and making appointments.
  • Assist in resolving health and social service issues including gathering verifications to assist in program eligibility determinations.

Need to have:
  • One of the following:
    • Two years or more of responsible, paid social service experience providing direct client service.
    • A two-year human services certificate.
    • Two years or more of college coursework in the behavioral sciences.
  • Ability to communicate fluently in Spanish in addition to English, both verbally and in writing.
  • Access to reliable transportation to travel to service sites within Hennepin County.
Nice to have:
  • Experience:
    • Working with the Spanish-speaking community.
    • Using databases and performing data entry where accuracy is essential.
  • Knowledge of community resources and social services available to clients.
  • Ability to:
    • Work in a fast-paced environment, multi-task, and manage time.
    • Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
    • Establish and maintain professional working relationships with clients, staff, and the public.
