Position Summary
As a Care Management Associate, you are responsible to provide a holistic, client-centered approach to caring for clients. The Care Management Associate provides assessments and continuous reassessment and comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence, and wishes are respected.
Minimum Qualifications

Three years of experience in community, hospital, hospice, or home health care management

Certified Care Manager (CMC) or Certified Case Manager (CCM)

Demonstrated knowledge of aging, dementia, and/or disabilities.

Experience working with complicated family systems and dynamics.

Excellent computer skills including MS Office, Electronic Health Records, and Internet Skills

Frequent local travel by personal automobile to client homes is required.
Preferred

Bachelor’s degree in Healthcare Related field
