THE PROGRAM

Centro La Familia Advocacy Services (CLFA) is seeking a qualified candidate to implement services for its Bringing Families Home (BFH) program in Fresno County. This program provides CPS families experiencing homelessness with rapid rehousing services that are tailored to meet their individual needs with the goal of achieving housing stability and increasing self-sufficiency. The BFH Housing Navigator / Case Manager will implement services that will help families and clients achieve long-term stability including goal planning, linkage to services, financial planning, and others. Services will be provided according to the principles of Housing First. This program is funded by the Fresno County Department of Social Services.

BASIC FUNCTIONS

The BFH Housing Navigator / Case Manager will work to build trust with families and implement trauma-informed, motivational interviewing approaches. Direct services include implementing assessments, developing case plans, providing linkages, and conducting regular follow ups. This position also requires the BFH Housing Navigator / Case Manager to attend meetings and trainings, and completing all reports as required. The BFH Housing Navigator / Case Manager will work closely with the Department Manager, and other staff to track services including housing costs, effective landlord engagement, financial planning techniques and other duties as needed.

QUALIFICATIONS

Candidate must possess excellent written and verbal communication, ability to follow-through, and be detail-oriented. Candidate must be organized, professional, and able to work in a fast-paced, multi-faceted environment. Candidate must be proficient in the use of technology to collect, analyze and manage program data as well as computer literate in Windows platform; proficient in Microsoft Word, Excel and Google Applications. Candidate must possess the ability to maintain good recordkeeping and documentation, good presentation skills, and have the capacity to be self-motivated. It is mandatory to meet employment eligibility requirements as established by the program’s funding agency.

ESSENTIAL JOB RESPONSIBILITIES

· Provide intensive support services to BFH families referred by Fresno County Department of Social Services.

· Engage with the referral homeless families to identify immediate and long-term housing.

· Assess family’s needs for additional resources beyond housing that will help them achieve long-term stability.

· Work to develop a case plan with clients and families.

· Implement intake forms and assessment tools including VI-SPDAT, HMIS, Family Development Matrix and others as identified and collect data as needed for reporting and meeting project goals and objectives.

· Provide linkages for families, including assistance with making appointments, transportation, appointment accompaniment, and/or providing a warm hand-off.

· Maintain knowledge of community resources including food distributions, transportation, clothing, medical care, employment services, mental health, and others.

· Maintain accurate client files, good recordkeeping, update as needed an adhere to confidentially policies.

· Participate in required trainings as needed.

· Develop a sense of belonging and teamwork among CLFA staff and collaborative partners including landlords to place clients in housing and or prevent homelessness.

· Organized, self-motivated, professional, and ability to work in a fast-paced, multi-faceted environment.

· Proficiency in the use of technology to collect, analyze and manage program data as well as computer literate in Windows platform; proficient in Microsoft Word, Excel and Google Applications.

· Empathetic, identifies and communicates effectively with diverse communities, in a culturally sensitive manner that emphasizes the importance of relationship and trust building between the client and advocate.

· Implement financial education and coaching in efforts to stabilize client’s situation.

· Monitor client’s progress and assist in resolving barriers to services.

· Must have a valid CA driver’s license and proof of insurance.

· CA Department of Justice finger print clearance.

· Performs other related duties as assigned.

Preferred Education/Experience

· Bachelors from an accredited college with a major in social work, social welfare, human services, or related field

· Minimum of two years’ experience in a community based human service agency as an advocate, navigator or other related work experience

· Bilingual abilities in another language common to the area is a plus but not required (English/Other language)

TYPICAL WORKING CONDITIONS: The office setting is a normal environment. Occasionally work during early morning, evening, or weekend. May me subject to temperature variances outdoor during events.

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, or walking for up to eight hours a day. Some bending, stretching, or reaching may be necessary. Lifting to 40 pounds may be required on occasion. Vision must be correctable to 20/20 and hearing must be in the normal range for telephone contact.
