Department Overview:
The Behavioral Health Resource Specialist works in partnership with the Behavioral Health Consultant to assist patients and their families to achieve maximum physical, social and emotional health. Provides resource/referral information about community resources including, but not limited to: rent and utility assistance, medical insurance, Medicare, Medicaid, SNAP benefits, medication assistance, and access to care. Assists families in accessing transportation and lodging resources. The person in this position must be able to demonstrate strong patient advocacy skills and follow patients through periods during which they may have intense multiple needs. This position requires innovative and creative thinking, self-directed work style, high level of energy, flexibility, time-management, communication skills, and drive. This position provides support to the OHSU Internal Medicine Clinic at Marquam Hill.
Function/Duties of Position:
Provides high quality service to assist patients in accessing local resources based on provider assessment and referral that meets or exceeds the service standards of the health care industry. This duty includes prompt and professional communication efforts, face-to-face and over the phone customer contact skills, crisis management, facility with available information, technology, and the continuous application of process improvement methods and skills. Refers patients to local resources based on their individual needs and assists patients in connecting with resources. Responsible to reviewing Epic Social Work pool on a daily basis, triaging referrals from medical providers and routine to socials workers as appropriate. Gathers and verifies patient information including demographics, insurance coverage, and financial status as needed to assess eligibility for resources and programs.
Assists Behavioral Health Consultant with administrative duties and general office tasks including retrieval and return of phone calls, electronic messages and faxes, making copies, completing paperwork and referral forms, creating marketing materials, and other activities as needed to support the Behavioral Health Consultant.
Maintains accurate and timely documentation of patient care activities in efficient manner per departmental policies. Completes all OHSU/Joint Commission mandatory education requirements and activities within established timelines. Yearly competencies as identified by department.
Participates in department meetings; participates in workshops, seminars and training sessions to maintain professional competency; assists in staff education; visits community facilities, programs or services to acquire pertinent resource information.

Required Qualifications:
Education:
A Bachelor's degree, or three more years of experience which shows: a) knowledge of community resources; b) knowledge of patient & family rights; c) knowledge of medical terminology; and d) knowledge of implications of the effect of illness, injury and/or disability on patients and families.
Experience:
Two years of direct patient and public contact on phones and/or in person providing customer service and assisting patients with accessing healthcare and other resources as needed.
Job Related Knowledge, Skills and Abilities (Competencies):
Intermediate level skills in Microsoft Word. Demonstrated ability to work with a variety of diverse customers in difficult situations. Skill in develop in and maintaining professional relationships. Ability to prioritize work demands. Ability to interpret and follow policies and procedures.

Preferred Qualifications:
Education:
Bachelor's Degree in Social Work (BSW)
Experience:
Experience within a medical service delivery system or health plan (Medicare, Medicaid).
Job Related Knowledge, Skills and Abilities (Competencies):
Experience working with an electronic medical record system.
