We’re seeking an experienced CA Licensed Clinical Social Worker with: · Master’s degree · Experience collaborating with healthcare providers in a multidisciplinary setting · Working knowledge of general theoretical and clinical models · Ability to develop rapport with patients · Excellent interpersonal and communication skills · Ability to document/chart to ensure current and accurate medical records · Knowledge of social work ethics and scope of practice · Ability to perform a complete mental exam and recognize changes in mental status · Knowledge of relevant community resources · Proficiency in coordinating care with providers and community agencies · Ability to serve as a patient advocate with assertiveness, persistence, and diplomacy
