Behavioral Health Counselor (BHC)

Position Summary:

Little River Medical Center (LRMC) is a Federally Qualified Healthcare Center (FQHC), seeking an experienced LISW-CP or PhD to join our team in our primary care setting serving patients of all ages and presenting with a wide range of behavioral health and medical conditions. Ideal candidates will have working experience and interest delivering brief, evidence-based treatments applied to behavioral health and medical conditions, and collaborating with primary care colleagues and clinical staff. Demonstrated proficiency in assessment and brief treatment in motivational interviewing, cognitive and behavioral therapy, trauma informed care, substance use disorder treatment, and chronic pain management are essential.

Qualifications:
  • Competent clinically in required job skills and knowledge (evidence-based practice and effective integration interventions); exhibits the ability to learn and apply new skills; keeps abreast of current developments; requires minimal supervision; displays understanding of how job relates to others; maintains confidentiality per policy and regulations.
  • Must be fully vaccinated against COVID-19
  • Must meet physical demands of the job; the work hours may vary with job demands.
  • Proficient in Microsoft software (Word, Excel, Outlook, etc.) and working within in EMR system
  • Minimum of 3 years’ experience in an Integrated Care/Primary Care/Family Medicine/Community setting preferred.
  • Master’s degree or Doctorate in Social Work or Master’s degree or Doctorate in Counseling required.
  • Full license to practice as LISW-CP in the State of South Carolina is required.
  • Must be fully vaccinated against COVID-19
  • Experience providing clinical experience for children and adolescents strongly preferred.
Description of Duties:
  • The BHC serves as a liaison between the patients, LRMC Primary Care Providers and clinical support staff, and community providers to ensure continuity of care and access to behavioral health needs services.
  • BHC will identify, consult, treat, triage, and manage primary care patients with medical and/or behavioral health problems utilizing team-based care.
  • Maintain a visible presence and is available for same day consultations with patients referred by our primary care providers.
  • Provides screening, initial assessment and diagnosis, including diagnostic impression and functional status on present problem(s).
  • Provide brief, behavioral, and cognitive, evidence-based interventions to address a wide range of co-morbid health, mental health, and substance use problems.
  • Assist with triaging and referring patients to specialty Mental Health/Substance Use Disorder (MH/SUD) services in the community when appropriate, supporting a smooth transition from MH/SUD services back to the primary care center level of care as necessary.
  • BHC will help patients reduce barriers to maintaining healthy outcomes in reinforcing individual goals to meet their health care needs.
  • Complete timely documentation in EMR ensuring sufficient feedback to primary care providers regarding consultation findings and recommendations.
