Responsibilities: The STAR Program provides integrated medical and behavioral health care at two locations in Central Brooklyn: University Hospital at Downstate and Brookdale Hospital Medical Center. Enroll patients into the program and conduct intakes; collect baseline data and screens; and develop comprehensive service plans. Provide case management, peer support and patient navigation, schedule medical appointments and services, link patients to CBOs for wrap-around”/recovery support services. Provide goal-oriented individual supportive counseling, on-site or via telehealth; provide group, family and couples counseling, in collaboration with LMSW/LCSW/LMHC. Utilize telehealth, texting and social media to engage and retain clients in care. Contribute to patients’ care plan development by regular attendance at case conferences. Record session notes and patient follow-up in the electronic health record. Participate in community-based outreach activities. Participate in ongoing quality improvement activities. Participate in the development of abstracts related to program outcomes and present on these outcomes when accepted by professional conferences. Minimum Qualifications: Bachelor’s degree in social work or related field with 2 years of experience providing counseling or mental health case management in a health care, substance use treatment center or community mental health care setting required; 2 years of experience providing counseling or mental health case management in a health care, substance use treatment center or community mental health care setting required; effective computer, verbal, organizational and interpersonal skills. Additional Preferred Qualifications: Lived experience or experience working with individuals with mental health or substance use disorders strongly preferred. Experience conducting psycho-social assessments, intakes and treatment plans, individual, family and group counseling. Training in Motivational Interviewing, Cognitive Behavioral Therapy (CBT) or other solution-focused evidence-based interventions preferred; CASAC or CASAC-T; experience working in multidisciplinary and multicultural, team-oriented setting; bilingual in English/Spanish or English/Haitian Creole an asset; experience using electronic health record; experience working with LGBTQIA+ populations. LMSW or LCSW may be substituted for 2 years’ experience. Salary Range: $50,000-$55,000 annually. Days/Hours: M-F, 9am-5pm; 37.5 hours/wk. Employer: The Research Foundation for SUNY.
