Primary Responsibilities

  • Provides individual case management under clinical supervision that meets productivity and quality requirements set by the Agency and that is within the limits of services permitted by position and scope of practice.
  • Accumulates the required number of billable hours of clinically allocated time based on Agency standards for position and profession.
  • Identifies Individualized Service Plan supportive services authorized by Utilization Management, provides linkage to said services within authorized duration and frequency, including working with Mental Health and Alcohol and Other Drug clinicians to develop multi-disciplinary services to ensure continuity of care.
  • Develops and maintains quality, accurate clinical charts for all clients served. This includes treatment plan development and case note documentation.
  • Develops and maintains positive collaboration with other social service agencies serving clients. Maintains knowledge and familiarity with referral sources within the community. Provides consultation when needed.
  • Serves as liaison between the hospital, criminal justice system and other treatment services in the community.
  • Other clinical duties as assigned.

Education, Certifications, and/or Experience

  • Bachelor's Degree required, preferably in areas of human or social services or other related field
  • At least one year of experience in substance use disorder treatment and case management services
  • Clinical licensure preferred but not required
  • Demonstrated competencies for working with the populations served
  • CPR/First Aid, de-escalation; TCN will provide upon hire
