Premier Health Associates is the region’s largest primary care based, multi-specialty medical group in Northwest NJ. We provide the patients of our community with comprehensive, patient-centered preventative care at 13 locations throughout Sussex, Warren, and Pike Counties.

We are looking for a motivated and compassionate Advanced Practice Nurse (APN)/Endocrinology that will work with our healthcare team to provide the best possible collaborative care to our patients in a full-time position. Candidates should be able to diagnose, treat, consult, and provide follow-up care while playing an integral part in the hospital diabetes clinic and will be involved in Inpatient Endocrinology under the supervision of an Endocrinologist.

Responsibilities:

  • Job duties will include diagnosing and treating both acute and chronic conditions.
  • Must be able to work independently, while working as part of the healthcare team.
  • Strong communication skills with a personable demeanor.
  • Develop a rapport with patients in order to promote preventive and longevity of care.
  • Collaborate with other health care providers in coordination of care and case management.
  • Conduct clinical assessments to develop appropriate treatment plans that include management of medication and referrals to other providers when necessary.
  • Emphasizes the importance of routine follow-up to ensure continuity of care.
  • Adhere to safety standards and protocols.

Qualifications:

  • Current unrestricted NJ license and certification as a Primary Care Advanced Practice Nurse.
  • Minimum or 2 years of professional nursing experience in Family Practice or an Ambulatory Primary Care Center.
  • Up to Date with all continuing education requirements.
