Now Hiring: Nurse Practitioners (Allentown, Bethlehem, Easton)


Hours: Per Diem


The Nurse Practitioner will be responsible for, but are not limited to:

  • Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
  • Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
  • Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
  • Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
  • Prepare and maintain accurate patient records, charts and documents.
  • Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
  • Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Qualifications:

Education:

Master's Degree in Nursing


Professional Experience:

  • Must have a Nurse Practitioner's license in Pennsylvania
  • Must have 2-4 years experience working within hospice or related field
  • Palliative care experience preferred
  • Must have a Master's Degree
  • Must have a valid driver's license and a reliable source of transportation
  • Must be willing to travel
