Reports to: LTCU Supervisor

FLSA Status: Non-exempt

Principal Purpose of the Job:

The principal purpose of this job is to develop and execute recreational and entertainment programs designed to meet, in accordance with the comprehensive assessment, the interests and physical, social-spiritual, mental, and psychosocial well being of each resident in accordance with the state and federal guidelines.

Essential Functions of the Job:

  • Plan, organize and coordinate a program of activities in and outside the L.TC. which will provide entertainment, inter-communications, exercise, relaxation, opportunity to express creative talent and fulfill basic psychological, social, and spiritual needs; which will be available to all residents’ subject to residents’ physicians written restrictions and/or limitations.
  • Develop and implement a monthly activity calendar that meets the physical, social, spiritual, mental, and psychosocial needs of the residents. Large type calendar will be posted in appropriate place for review by the residents and the public.
  • Encourage hobbies and craft projects-provide materials/supplies in the keeping with the resident’s financial status and department budget.
  • Plan trips (shopping, scenic, Olympic games, eating out, school play, etc.) and arrange transportation and supervision.
  • Encourage and assist residents to participate in religious activities to fulfill spiritual needs through visits with clergy, attendance at religious services within the facility and at their home church when possible. Provide spiritual reading material (Guide posts, upper room, bible, etc.).
  • Provide reading material in braille, large print, and tapes/records for vision impaired residents; in conjunction with library services for all residents.
  • Plan and implement one on one activities for room bound residents.
  • Plan and implement appropriate individual activities for residents who are unable or unwilling to participate in scheduled activities.
  • Recruit and orient volunteers.
  • Supervise volunteers in scheduled activities and L.T.C.
  • Work with Wichita County Health Center Volunteer Committees.
  • Participate as a team member in helping to maintain physical, social, and psychological environment conductive to the resident’s welfare and best interest.
  • Do an activity assessment upon admission. Update annually, hospital update or significant change.
  • Write an activity plan of care. Review every 90-days, hospital update or significant change.
  • Activity progress notes to be written every 90-days after review of plan care.
  • Maintain a daily record of resident’s attendance at activities (check sheet).
  • Maintain a record of any other documentation as mandated by state and Federal regulations.
  • Assist in developing the MDS+ (Section G). Review every 90-days. Do Raps and Triggers as indicated by review.
  • Attend Plan of Care Meetings as scheduled by care plan coordinator.
  • Attend required meetings – Department Head meetings, LTC Supervisor meetings, and all other meetings, as assigned.
  • Participate in on going Quality Assurance Program.
  • Interview swing bed patients within 48 hours of admission for activity and supplies.
  • Follow fire and safety procedure.
  • Report any accident/incident or hazard.
  • Follow established infection control and universal precaution policies and procedures.
  • Adhere to all Resident’s rights, including privacy, confidentiality, and treat residents with dignity and respect.

Additional Responsibilities:

· Participate in training and education as requested and complete Care Learning courses annually.

· Come to work as scheduled and consistently demonstrate dependability and punctuality.

· Cooperate and work together with all co-workers.

· Promote positive public relations with patients, family members, and guests of WCHC.

· Uses tactful and appropriate communication in all situations.

· Report to work in neat, clean attire and consistently present an appropriate professional appearance.

· Assume accountability for data contained in the employees’ handbook

· Promote and support safety awareness and implement safety measures.

· Cooperate to investigate employee injuries.

· Share knowledge with others. Act as a role model and mentor to new staff.

Physical Requirements:

This position demands good physical and mental health. The work throughout the facility, including the residents’ rooms. Sitting, standing, bending, lifting, and moving intermittently during working hours. Involvement with residents, personnel, visitors, resident’s family, and at times government personnel. Is subject to physically ill residents as well as hostile and emotionally upset residents. May be required to work beyond normal working hours, on weekends and holidays. Minimal risk to exposure to infectious waste, diseases, conditions including HIV and Hepatitis B viruses.

Qualifications, Education and Experience:

Must possess, as a minimum, a high school diploma or equivalent.

One or more years of long-term care or related experience preferred.

Must possess Kansas Activity Director certification.

Must be able to write, speak, understand the English Language well enough to communicate with residents, receive written/oral directions, and complete essential job functions.

Must be able to see and hear well enough to implement planned activities and to communicate with residents and staff.

Must Follow Instructions and accept constructive feedback.

Must be able to function independently, be flexible to work harmoniously with residents, staff, and visitors.

Must be able to relate to physically ill, mentally ill, and at times hostile/assaultive individuals within the facility.

Must possess aptitude in arts and crafts.

Must possess leadership skills and be wiling to cooperate with other departments and function as a team. Must have an understanding of social, psychological, and recreational needs of the aged infirm and disabled.

Must have the ability to plan and organize activities geared to resident’s needs.

Must have patience, tact, a sense of humor, and imagination.

Must be able to evaluate residents and set realistic goals according to their level of functioning.

Must possess a current driver’s license.
