Berkshire Healthcare Systems’ newest affiliate, East Longmeadow Memory Care Assisted Living Center opens in June 2022. This 20 bed residence is just steps away from the state-of- the-art Skilled Nursing Center which was built in 2020 providing a supportive Campus of Care for the local community. Our memory care assisted living community provides a unique environment where residents receive personal assistance with activities of daily living in a supportive environment for one price that does not escalate with higher care needs.

At East Longmeadow Memory Care Assisted Living, our Life Enrichment Memory Care Program (LEP) is designed for those who need support with their daily activities such as bathing or dressing. When care needs progress, residents may transition to our certified memory care neighborhood at the Skilled Nursing Center next door. Our secure unit is designed to create a familiar transitional environment from the assisted living to long term care in order to minimize confusion and provide comfort.

East Longmeadow Memory Care Assisted Living and Skilled Nursing Center are conveniently located on the border of Longmeadow and East Longmeadow at 305A & 305C Maple Street, East Longmeadow.

Job Description:

The primary purpose of this position is to plan, organize, develop, and direct the overall operation of the Activity Department in accordance with current federal, state, and local standards, guidelines and regulations governing the facility.

Essential Job Functions:

  • Plan, develop, organize, implement, evaluate, and direct the activity programs of the facility.
  • Assist in the development, administering, and coordinating of department policies and procedures.
  • Keep abreast of all regulations & professional standards, & make recommendations on changes to the Administrator.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Participate in discharge planning, development and implementation of activity care plans and resident assessments.
  • Perform administrative requirements, such as completing necessary forms and submitting such to the Administrator as required.
  • Involve residents and families in planning facility activity programs.
  • Assist in arranging transportation to other facilities when necessary.
  • Refer resident/families to social service personnel when the facility does not provide the services or needs of the resident.
  • Provide consultation to staff, community agencies, etc., to solve the needs of residents through the activity programs.
  • Review and evaluate the department's work force and make recommendations to the Administrator.
  • Coordinate activities with other departments as necessary. Berkshire Healthcare Systems, Inc.
  • Develop a plan of correction for activity deficiencies noted during survey inspections and provide a written copy to Administrator.
  • Review departmental complaints from personnel and report to the Administrator action(s) taken.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Arrange transportation for field trips when necessary.
  • Participate in various committees of the facility as required, and as appointed by the Administrator.
  • Attend department head meetings, etc., as scheduled or as may be called.
  • Assist in the recruitment, interviewing, and selection of personnel for the activity department.
  • Determine staffing requirements to meet the department's needs, & assign a sufficient number of staff for each tour of duty.
  • Make rounds to assure personnel are performing required duties, and activity programs are meeting the needs of the residents.
  • Conduct departmental performance evaluations in accordance with the facility's policies and procedures.
  • Encourage residents to participate in hobbies and crafts. Provide materials as necessary.
  • Develop and maintain an activity schedule.
  • Others as deemed necessary and appropriate, or as may be directed by the consultant or administrator.

Qualifications:

  • Must possess, as a minimum, two (2) years of college. Degree preferred but not necessary.
  • Experience in a health care setting, facilitating an activities program for people with dementia preferred
  • Experience in a social or recreation program within the last five years, one (1) of which was full-time in a patient activities program in a health care setting
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing activity services.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
