Women's Health Nurse Coordinator (MBCCP)

YOUR ROLE & IMPACT

As part of a dynamic team at Charles River Community Health (CRCH), a healthcare practice providing compassionate care to underserved communities, the Women’s Health Nurse Coordinator is responsible for driving value based care initiatives. The Women’s Health Nurse Coordinator will integrate clinical programs and resources into the practice workflows with the intent to optimize patient care delivery to our patient population, improve the provider and care team experience, and enhance quality of care.

YOUR RESPONSIBILITIES

  • Function effectively and safely to strengthen the practice and provide patient-centered care.
  • Will lead the MBCCP grant and required activities by providing culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.
  • Collaborate with Mammography Coordinator, Case Managers, and Women’s Health Coordinator, in conjunction with the Practice Transformation Director, to coordinate program and achieve all required deliverables.
  • Enhance client outcomes and reduces barriers to care. Initiates and implements plan through monitoring and re-evaluation of program deliverables.
  • Understand patient conditions and treatment pathway. Monitors plan of care, as indicated.
  • Demonstrate knowledge of community resources and assist staff, patients, and families to effectively utilize them. Ensure effective coordination of care across the care continuum, including Community Based Organizations.
  • Manage care for complex patient cases, assisting effective and timely follow-up and monitoring of abnormal results.
  • Develops partnerships with Community Based Organizations. Facilitates and attends meetings between CRCH and community partners. Facilitate outreach opportunities and coordinate community events for MBCCP patient enrollment.
  • Establish rapport and a relationship with the patient and family in order to understand their needs and expectations and to assist them in setting realistic and mutual goals. Integrates an awareness of cultural factors in the patient/family interview process and elicit clinically relevant cultural information.
  • Use QI tools and methodologies to assess performance and identify gaps between current state & best practice related to key clinical outcome measures.
  • Serve as a resource person to other members of the team around the care needs of patients, and provide support to other members of the multi-disciplinary team in an accessible, open manner. Delegate safely and effectively to support staff tasks that are within scope of practice.
  • Foster communication and collaboration that presents opportunities to deepen your knowledge about specialty areas.
  • Organize, manage, and analyze grant data, as required.

YOUR QUALIFICATIONS COMPETENCIES TRAITS

  • Bachelor of Nursing preferred.
  • Must have Massachusetts RN license.
  • Minimum 2 years of experience working with population health or women’s health
  • Must have case management experience.
  • Must be able to lead and manage program.
  • Ambulatory care experience preferred
  • Data management/analysis preferred
