Position: Care Management Concierge Nurse Ambassador

We provide compassionate, virtual assistance to elderly patients utilizing advanced remote patient monitoring and sophisticated care management protocols. The system is very simple. Our partner physicians give us EMR access and we identify and enroll patients who are eligible. We send them cell-chip enabled devices and manage them utilizing clinical algorithms to triage device alerts and provide a comprehensive whole person management program built by our founder, Dr. Jeff Gruen, a nationally recognized pioneer in population health management.
All of the care is virtual so you can work from the pleasure of your own home, but at the same time have the benefit of providing sophisticated care that will challenge you and keep you sharp as part of a great team.

We are generally considered the best company in our sector in the country and were selected by
Oracle/Cerner as their sole partner for their 1600 hospitals.

Your job as Care Management concierge Nurse Ambassador will be to work closely with your own
“practice” of 100-125 patients. You will get to know them well, and will work with them and their families doing everything you can to keep them healthy and help them improve their outcomes. The patient comments are so motivating, they feel for the first time someone is truly looking out for them.

You will also work as part of a team that you will be responsible for directing, including two medical assistants who will leverage you and do certain components of the work.

What’s cool about this job is that you will be constantly evolving, finding new and better ways to help the patients within a clear set of guidelines and frameworks because we are passionate about making a difference.

You will be measured by the quality of the interactions you produce, the strength of the relationships you create, the outcomes you generate, and the efficiency with which you provide the service.

You can expect training and then hitting the ground running within a few days of start, with the expectation you will fully assume your case load within the first couple of weeks, all with the support of a team around you.

Compensation is competitive, equity participation in the company is important.

LPN or RN license necessary. Only Florida, California and Ohio currently needed, but future needs will evolve. Previous care management experience a plus but not critical --- good solid nursing, empathy and

thinking skills are critical for success. We are very inclusive. Bi-lingual English/Spanish is extremely important although not 100% necessary.

Job Types: Full-time, Part-time

Pay: $28.00 - $50.00 per hour
