Saint Simons By-The-Sea Hospital is seeking a FT Therapist for inpatient to assess and counsel patients regarding psychological or emotional problems and develop and implement therapeutic treatment plan.
Responsibilities include, but are not limited to, the following tasks:
Perform psychosocial assessments and compile all pertinent documents to facilitate treatment planning on assigned patients.- Participate in treatment team sessions with completion of all treatment plan updates.
- Monitor the progress of all patients assigned and update treatment plan, as necessary.
- Conduct group and/or individual sessions as assigned.
- Perform group/individual therapy and document progress notes.
- Respond to any emergency situation involving patients in treatment.
- Participate as a team member by cooperating with staff and demonstrate compromise and expression of opinions as appropriate.
- Utilize chain of command in all aspects of program and follows organizations policies and procedures.
- Contact and schedule counseling sessions with family, significant others, and involved agencies as appropriate. Ensure that there is family contact within 72 hours.
- Attend staff meetings and in-services as required and perform duties as a positive role model for clients and colleagues.
- Accurately, and efficiently, assess patients' aftercare needs in collaboration with the patient, physician, and treatment team.
- Provide patients with reasonable aftercare options and discuss each option in full with the patient individually, and/or with family as requested by the patient.
- Collect releases of information to accurately research and refer to services.
- Initiate contact with referral sources, community mental health providers, physicians, nursing homes, residential treatment facilities, halfwayhouses, and other placement options to facilitate continuity of care.
- Schedules follow-up appointments with a psychiatrist and therapist within a reasonable time following discharge (Psychiatrist: 30 days and Therapist: 7 days).
