Blaze Health North Memorial Health Clinics (Minneapolis, MN) is seeking a dynamic advanced practice provider (Physician Assistant or Nurse Practitioner) with a minimum of three (3) years direct patient care experience, to join our team as a Telehealth provider. This is a remote position that is an extension of our Primary Care and Urgent Care clinics. Our ideal provider is someone motivated to provide high quality, customer-centered care remotely, via Telehealth, and functions as a member of the care management team.

Position Purpose

The Telehealth advanced practice provider (APP) uses technology to deliver high quality care and provide virtual medical services and professional guidance and support to customers via video. The Telehealth APP will assess customer needs, gather important clinical history, diagnose, provide appropriate clinical guidance or treatment, and document findings. Our Telehealth APPs are members of the patient care team, helping to ensure that the delivery of quality healthcare is given to all our customer. Whenever possible, shared decision making is used with our customers in order to meet our customer expectations.

Qualifications:

Physician Assistant

• Current Minnesota Licensure as a Physician Assistant through the Minnesota Board of Medical Practice.
• Physician Assistant Certification through National Commission on Certification of Physician Assistants (NCCPA).
• Valid DEA Registration
• Resident of Minnesota or Wisconsin

Nurse Practitioner

• Current licensure as RN in State of Minnesota
• Current registration as a nurse practitioner with the MN Board of Nursing
• Valid DEA Registration
• Resident of Minnesota or Wisconsin
