We are seeking a Social Worker for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. What you will do in this role:

  • You will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.

  • You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

  • You will identify potential barriers to patient throughput and quality outcomes

  • You will evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.

  • You will act as a liaison between the facility and resources external to the organization.

  • You will help with the facilitation and implementation of timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.

  • You will provide crisis intervention and support to patients/family support systems.


  • Licensure as a LMSW is required. LCSW is highly preferred

  • Master's degree in Social Work Required

  • 3-5 years experience in hospital patient care preferred
