What Will you be Doing?
- Assess the psychosocial status of the patient related to the patient illness and environment and communicates findings to the clinical care team.
- Assists the health care providers in understanding significant social and emotional factors related to the patient’s health problems in order to develop and update the plan of care as indicated.
- Provides and documents services in accordance with state, Medicare, Medi-cal and other applicable regulations and standards and third party payer guidelines.
- Uses appropriate infection prevention control, universal precautions, procedures and safety measures.
- Assesses patient and family and prepares social histories to augment existing service, or as a guide in determining or changing level of service.
- Assesses, when appropriate, a patient/family’s financial situation taking into consideration patient’s prognosis and medical needs, referring to an agency for financial assistance when indicated, interpreting the medical situation to the referring agency and generally facilitating the process of rehabilitation.
- Assists patient and family with personal and environmental difficulties that predispose toward illness or interfere with obtaining maximum benefits from medical care or cope with terminal illness.
- Serves as a liason between patients and families to community agencies to utilize available resources to rehabilitate the patient or assist the patient to live with the disability or terminal illness.

Salary Range
$35 - $55 / hour
Minimum Requirements:
- Master’s degree in social work from a school of social work accredited by the Council on Social Work Education.
- One Year experience in health care setting, experience in home health preferred.
- Licensed in the state of practice if required (in CA, MSWs are not required to be licensed if in non-supervisory role).
- Thorough knowledge of community resources.
- Must have a vehicle, current driver’s license and appropriate insurance coverage.
- Communication, interpersonal, and organizational skills necessary to complete job responsibilities.
- Ability to make home visits.
