The primary purpose of this position is to plan, organize, develop, and direct the overall operation of the Gilbert Residence's Social Service Department in accordance with current federal, state and local standards, guidelines and regulations of our established policies and procedures, and as directed by the Administrator, to assure that the medically related emotional and social needs of the residents and families are met and maintained on an individual basis.

DUTIES

Administrative

1.Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.

2.Participate in discharge planning, development and implementation of social care plans and resident assessments.

4.Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.

5.Assist the Quality Assessment and Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.

6.Schedule and facilitate care conferences for residents, families, and staff.

Clinical

1.Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident's response to service.

2.Develop preliminary and comprehensive assessments of the social service needs of each resident.

3.Develop a written plan of care for each resident that identifies social problems/needs of the resident and the goals to be accomplished for each problem/need identified.

4.Ensure that social services components of the MDS are completed and signed on a timely basis.

5.Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.

6.Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

QUALIFICATIONS

Education

1.Must possess, as a minimum, a Bachelors Degree from an accredited school of Social Work.

Experience

1.Must have, as a minimum, one year of supervised social work experience in a health care setting working directly with patients/residents.
