Role:

We are seeking a dedicated Social Services/Admissions Nurse Manager to join our Colonial Village team! The Admissions Coordinator makes assessments for, plans, and advocates for the residents of the community facility so they can reach their highest practical level of physical, mental, and psychosocial well-being. The Admissions Coordinator will be responsible for the pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident-centered/directed care to team members, families, physicians, allied health professionals, and the at large community.


Shift:
This position is part of the Nursing Management Team and will regularly work Monday through Friday with rotating on-call and Manager on Duty.


Responsibilities:

  • Social Services/Admissions Nurse Manager ensures that the highest quality standards for resident advocacy are maintained.
  • Social Services/Admissions Nurse Manager collects and assesses data relevant to Elder’s psychosocial needs, risk factors for psychosocial deterioration, and responses to interventions.
  • Social Services/Admissions Nurse Manager completes required forms and documents in accordance with company policy and state and/or federal regulations.
  • Social Services/Admissions Nurse Manager coordinates admissions and discharge planning.
  • Social Services/Admissions Nurse Manager coordinates and directs the care plan meetings.
  • Social Services/Admissions Nurse Manager facilitates the advanced directive decision-making process.
  • Social Services/Admissions Nurse Manager supports Elder advocacy with a leadership role in Resident and Family care plan meetings.
  • Social Services/Admissions Nurse Manager coordinates grievance complaint process.
  • Social Services/Admissions Nurse Manager resources the mental health community to provide ongoing support to the Elders at Colonial Village.
  • Social Services/Admissions Nurse Manager coordinates and assist with resourcing and scheduling of allied health services i.e. Dentists, Podiatrists, Medical specialists, etc.
  • Social Services/Admissions Nurse Manager provides ongoing education of Resident’s rights to staff, family members, and the community at large.
  • Social Services/Admissions Nurse Manager be the facility educator for Abuse and Neglect Prevention Mandatory Training
  • Social Services/Admissions Nurse Manager comply with standards of confidentiality as defined by HIPPA
  • Social Services/Admissions Nurse Manager helps develop relationships with community resource groups e.g. Alzheimer's Associations, Church groups, and support groups.
  • Social Services/Admissions Nurse Manager perform other department duties or special assignments as directed.

Qualifications:

  • Must be a graduate of an accredited school, be currently licensed RN or LPN in the state of which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements.
  • Must either have a B.S. in the Social Services field or have 2 years of experience working as an SSD in a geriatric setting.
  • CPR certification is required.
  • Prefer 2-year experience working in a geriatric setting.
  • Must possess a valid driver's license and have a good driving record.
