Martin Avenue Apartments, an independent senior living community in the Naperville Area is hiring a Part Time Social Services Coordinator !!

SUMMARY

This position is primarily responsible for coordinating home and community-based services for residents and their families and provides social service intervention as needed by performing the following duties:

DUTIES & RESPONSIBILITIES:

• Perform Resident Needs Assessments and maintain resident records.
• Assess residents who may require assistance in daily living activities or who require a higher level of care. Involve family and other professionals, as appropriate.
• Make referrals to community service agencies and follow up on service.
• Coordinate with hospital staff regarding discharge planning for residents returning to their apartments.
• Provide crisis intervention with residents on a short-term basis.
• Provide case management services for residents receiving services from various community agencies.
• Advocate for residents.
• Maintain current knowledge base of services and programs impacting seniors and/or disabled to enhance ability to assist residents.
• Identify health and education programs to enhance the residents’ quality of life.
• Establish linkages and maintains contacts with community agencies offering services to residents.
• Develop and maintain Community Resource Directory.
• Submit required monthly/quarterly reports in a timely manner
• OTHER SKILLS & QUALIFICATIONS
• · Ability to effectively communicate with residents, community organizations, and staff.
• · Understand social services funding and eligibility requirements.
• · Ability to work effectively as a team and interact in a professional manner.
• · Effectively apply principles of logical thinking in defining problems, drawing conclusions, and initiating appropriate action.
• · Excellent oral and written communication skills.
• · Social Service software skills.
• · Experience with Microsoft office.

EDUCATION

• Bachelor’s Degree (Desired) or Associates Degree in Social Work, Counseling, Sociology or Social Science.
• Minimum two Years of related work experience
